Naturopathic Medical Intake and Evaluation: 

Expect 1.5-2 hour visit with the physician to talk at length about your health history and current issues, including lifestyle and dietary habits. Vitals and physical exam done as well. Please bring a list of your medications and vitamins/supplements that you take and any recent lab work or imaging that is relevant.
